Communication Policy

  • Communication platforms used: email, text message, and FaceTime

  • Questions that require more than a 15min answer are invoiced

Cancellation Policy

  • For personal training, cancellation is within 24 hours of the appointment, otherwise a $25 fee is charged. For online programming, you will get a full refund of your money if you are not satisfied with the program.*

    • Completed programs exempt from this refund.

What information do I collect?

During your consultation I collect the following information:

  • Your name

  • Your address

  • Your contact information including email address and phone number

  • Demographic information 

  • List of current and past medications

  • Medical history (including surgeries & injuries)

  • Nutrition questionnaire

  • Motivation assessment

  • Lifestyle questionnaire 

  • Body composition test (depending on program)

What do I do with this information?

I require this information to understand your needs and provide you with a better service, and in particular for the following reasons:

  • Internal record keeping.

  • Improving my service to you.

  • I use medical records to better customize your program to fit within your abilities.

  • I may periodically contact you by phone, email, text message or mail to inform you about new products, special offers or other information which we think you may find interesting using the information you have provided.

  • From time to time, we may also use your information to contact you for market research purposes.
